UT - Lead Certification - Lead Abatement Worker

COURSE DURATION 16 hours
PREREQUISITES -Notarized proof of citizenship form and copy of drivers license or green card (for first-time application only) -Successfully complete an accredited course in the appropriate discipline and receive a course completion certificate from an accredited training program -No additional experience and/or education requirements
ACCREDITING BODY Utah Department of Environmental Quality
INDIVIDUAL APP REQUIREMENTS -$100 for one year -$200 for two years -$300 for three years
INDIVIDUAL EXAM FEES No third party exam
COMPANY APP REQUIREMENTS $250 each year, can pay up to a maximum of five years
REFRESHER TRAINING -Every 3 years if the individual completed a training course with a course test and hands-on assessment -If more than 3 years but less than 4 years have passed since certification or re-certification for an individual that completed an initial or a refresher training course with a course test and hands-on assessment for the supervisor, inspector, and/or risk assessor disciplines, then the individual must also pass the certification exam in the appropriate discipline offered by the director
RECIPROCITY There are numerous reciprocity agreements with EPA and other states with authorized lead-based paint programs. Your state may accept training for certification programs from those training providers accredited by EPA or other state authorized programs. Because reciprocity agreements are subject to change, it is strongly recommended that you contact your state's accrediting body before taking training from an outside source.
