COURSE DURATION	24 hours
PREREQUISITES	None
ACCREDITING BODY	EPA
INDIVIDUAL APP REQUIREMENTS	$410 every three years
INDIVIDUAL EXAM FEES	Third party exam. $70 for each attempt.
COMPANY APP REQUIREMENTS	$550 initial and $35 for each additional jurisdiction
REFRESHER TRAINING	Every 3 years if the individual completed a training course with a course test and hands-on assessment; OR every 5 years if the individual completed a training course with a proficiency test
RECIPROCITY	To apply for certification from EPA if you are currently certified by an authorized state, U.S. territory, or Indian tribe, you must: Complete, sign, and date EPA form 8500-28 (Rev 09/04) Enclose a copy of your valid certificate from an authorized state, U.S. territory, or Indian tribe, highlighting the certification expiration date, if applicable Submit two identical passport photographs of you alone Calculate the appropriate fee using the fees schedule provided
